Management of a Large and Aggressive Bone Disease (CGCG) Affecting the Lower Jaw

Teerthanker Mahaveer University’s Department of Oral & Maxillofacial Surgery (TMDCRC), recently performed an interesting and challenging tumor surgery that had almost eaten up 40%  of the lower jaw bone of the patient. The surgery was performed by the senior consultant Oral & Maxillofacial Surgeon Prof. and Head Dr. Arunkumar KV along with his team of faculty and postgraduate students.

The 55yr old female patient was suffering from a rapidly growing tumor inside the mouth from the last 6 months. She had bleeding gums with pain, bad breath and felt week due to loss of appetite. After a thorough investigation and biopsy, the case was diagnosed as Central Giant Cell Granuloma (CGCG), a bone disease that causes increased bone resorption. The treatment plan included surgical resection and reconstruction under general anesthesia (GA).

Under GA, with a combined extraoral and intraoral approach the tumor was excised completely by extended marginal mandibulectomy and bone debridement. The weakened lower jaw was supported with a reconstruction plate to avoid bone fracture. Chemical cauterization of the excised defect was performed and reconstructed with pedicle submental flap transporting intraorally. The surgery was completed without any complications. The patient has recovered completely with good wound healing.

These kind of aggressive CGCG tumors once treated need regular follow up as there is a chance of recurrence by 15-20%.

TMU’s dental college allows the students from the post-graduation (PG) and final years UG students to participate and view such operations for better learning on a regular basis. Professor Dr. Arunkumar KV also expressed the need for exposure to such surgeries to the students doing the internship and motivates them to choose this elite specialty which not only believes in curing diseases but also reconstructs them for a better quality of life.
